A whole webpage is devoted to the Jacobs 104 Convertiplane on Prototypes.com, Jean-Philippe Colliat's excellent website:

You can learn there that the project was cancelled during initial flight testing of the prototype. It is not known if the aircraft actually transitioned in flight. The program's assets were purchased by Vanguard and some mechanical elements such as the rotors and transmission were incorporated into their Omniplane prototype.
