Do you reallt think that missile with several km CEP (by the way, the most common data is that max CEPwas about 5 km) could be used against ship formation?

Yes, why not?

The X-22 flight time at max range is about 10 minutes. In ten minutes, the 30-knot warship could move up to 9 km, thus going out of disabling range even for megaton warhead. In general, Cold War era warships required abou 20 psi of shockwave overpressure to be disabled. Which means that even megaton warhead must hit within 3 km of the ship.

Max range has no anti ship value, Kh-22 requires to be locked on before launch, which limits range to what curvature of Earth allows you (unless it's a Fjord, but no one is running 35 knots in a Fjord).
This realistically limits the range to 350-400km, and as such, the flight time is within 5 minutes.

Also, missile is launched into a predicted point, rather than into the last known one. And yes, it can miss; all weapons can.
