Losing a wheel is not rare. It's not common, either, but it does happen.

My guess is that the cotter pin sheared and the retaining nut worked its way off, then the tire walked off the axle. Aircraft wheel/axle assemblies are very similar to an unpowered car axle with a single center knock-off nut.
 
Its the dialog that cracked me up with describing the path and hunt for the wheel, and 'tug' having a rather different slang meaning in Britain.
