This happens anywhere there's people. There's a few personality types that will always default to hiding and ignoring a problem no matter how severe the consequences or how high you stack the training, rewards, and penalties the other way on top of those consequences.

I'm going to laugh my ass off if it turns out we are talking about the same event from different sides.

But dude.

How TF can a cook allow the ship to run out of coffee of all things?!? Gotta wake up Chief for some BS, bring a peace offering. Basic survival skills, man!

We had a pretty "special" division at the time. A couple of the guys were really good, but we had some real knuckleheads too. And the CSC we had at that particular time was getting short and didn't really give a crap anymore.

Fortunately, once his relief showed up things turned around and it was like night and day. Our cooks were fantastic.
