This is apparently the much discussed conventional Trident SLBM
I think not. The linked articles all describe it as an
alternative to the Conventional Trident Missile, which COngress was unahppy with due to verification issues. The precise technical solution for CSM has never been described, I believe. However, I did come across one interesting article suggesting that the upper stage would be derived from DARPA's Hypersonic Test Vehicle project.

Here's a brief discussion of HTV-2, which appears to be close to the upper stage envisaged for CSM. This is not FALCON/Blackswift--HTV-2 seems to be much less ambitious.

Based on these two articles, CSM might be something like an Orbital Sciences Minotaur launch vehicle with a hypersonic maneuvering warhead vehicle as the final stage. Given that Minotaur is basically a space launch vehicle built from the three lower stages of an MX/Peacekeeper (LGM-118) plus a commercial final stage, turning that back into a missile isn't much of a stretch.
