737 motor fail!

Greetings
737 today had motor failure,speculation what caused it and whats missing on it?
Occurred in midflight,landed okay. ;D

seems that the CFM International engine had some problem again

That type of engine had issue with thrust instability, engines flaming out, Fuel flow problems and Fan blade failure.
here it blow off it's entire Engine inlet

certainly this accident will be in next season of "Mayday" and "Air-crash Investigation"

I'd say it's more a case of careless ground staff leaving the engine cowling unlatched. The AAIB report below covers an incident involving an A319.

Not sure if CFN56 has latched cowling sections at the area that went to daizies
 
You can see the fan is still there and it just looks like the intake/cowl has detached.
